Dr. Lewis is “in Network” with the following Insurance Carriers:

United Concordia, Aetna Extend, and Cigna

We accept all PPO insurance. We do not accept any HMO or DMO insurance

Financial Policy

Dr. Lewis would like to make our services available to everyone.
We offer a range of payment methods that include:

Cash, Checks, American Express, Visa, MasterCard, Discover

Since many elective cosmetic procedures aren’t covered under insurance plans,
Dr. Lewis also offers several convenient financing options to our patients. 

Cancellation Policy

If you are unable to keep an appointment, we ask that you kindly provide us with 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient. All cancellations after the required time will be subject to a charge.
